Group Employee Benefits Claims

There are two ways to submit a MEARIE Group Employee Benefit Claim.

Online Submission - Submit claims online and arrange for direct deposit claim payments - paid directly into your bank account - by clicking here to Great-West Life's GroupNet for Plan Members.

Paper-based Submission - To submit a Group Employee Benefit claim, please click to access all of the necessary medical forms. These forms have contact information and helpful tips specific to each type of claim. To ensure your claim is processed without delay, please include your:

  • Name
  • Date of birth
  • Policy/Plan number
  • Employee Identification number 

Remember to sign and date the form and submit it to the address on the Claim Form. KEEP A COPY FOR YOUR RECORDS.
